EMERGENCY MEDICAL FORM

A completed Emergency Medical form is required to participate in all NCCS sports, if your athlete is currently enrolled at NCCS, this was completed during the enrollment process. By checking the box below you are acknowledging that you have a completed Emergency Medical form on file at NCCS for the current school year. If you are a Home School Family, please check the appropriate box below and complete the Emergency Medical Form in the Home School section.

INSURANCE

NCCS carries insurance as required by the State of Oregon. This insurance is provided by the school as a secondary carrier. Student health and accident insurance is not included in this coverage. Such personal insurance is the responsibility of the family. All student athletes must have accident and health insurance to participate in a NCCS sport. Please enter the following:

SPORTS PHYSICAL FORM

NCCS requires a new physical every two years for all athletes, please return completed form to the main NCCS office.

5th - 8th: Sports Physical form from athlete's physician.

9th - 12th: OSAA Physical form required (see below).

14. Home School Family Requirements

I have read the NCCS Mission Statement. I will drop off a completed Home School Eligibility Form to the NCCS office (HS only) I will return a completed Emergency Medical Form to the NCCS office. I will return a completed Oregon Certificate of Immunization Status form to the NCCS office. NCCS currently has a complete and updated immunization form on file.
